Incline Health Management LLC owns and manages the brand, intellectual property, and administrative infrastructure; it does not provide medical care or make clinical decisions.

Pricing and Separate Costs

Incline+ Oral has a $79 monthly clinical care membership; prescription medication costs are separate. Incline+ Injectable combines medication and clinical management in an all-inclusive package without an additional monthly membership fee; package prices and enrollment availability are not yet confirmed. FDA-approved branded injectables may be discussed upon request; applicable costs will be explained before treatment. Any required lab costs remain separate. Published prices and manufacturer programs may change and may depend on eligibility and other program terms. Information on this website does not guarantee a particular price, insurance coverage, reimbursement, or approval of a prior authorization.

These website Terms do not enroll you in care or create a payment obligation. Any patient consents and clinical or financial agreements are handled separately through the clinical enrollment process.
